Class Liability Waiver Form

RELEASE/WAIVER: I hereby agree to indemnify and hold harmless CRAFT IT YOURSELF (CIY) and its employees at 125 S BROADWAY COWETA OK 74429 against all claims for personal injuries or damages of any kind arising from participation in any Candle-Making workshop, specialty classes, and their resulting products.

NOT MEDICAL ADVICE: I understand that no ingredients, ingredient descriptions, or guidelines presented are intended to treat, prevent, or cure any disease or condition.

PHOTO CONSENT: By your signature, you agree that CRAFT IT YOURSELF (CIY) may use the below named participants photograph in the routine promotion of its activities.

SUBSEQUENT VISITS: This waiver shall be kept on file and adhered to for both initial and subsequent visits to the workshop by the participant. You may revoke this waiver at any time by sending a statement via certified mail to: CRAFT IT YOURSELF 125 S BROADWAY COWETA OK 74429. I fully understand and acknowledge that the workshops offered at CRAFT IT YOURSELF (CIY) may involve inherent risks, dangers and hazards which may result in injury. All Candle-Making Classes & Workshops offer a No Refund Policy.

HAZARD ACKNOWLEDGEMENT: Due to the potentially dangerous nature of wax at high temperatures which may rise up to 200 degrees Fahrenheit and hot equipment including but not limited to metal wax melters and pouring pots, it is the responsibility of the participant to ensure the proper placement of their skill level. CRAFT IT YOURSELF and its employees cannot accept liability for any personal injury or damages that the participant may suffer related to or involving the premises of the event or participation in activities sponsored by CRAFT IT YOURSELF. Every effort is made by CRAFT IT YOURSELF and its employees to ensure the participant's safety while at the event premises and during the course of the workshop.

I fully understand and acknowledge that the workshops and/or activities offered by CRAFT IT YOURSELF may involve inherent risks, dangers and hazards which may result in injury or illness. I hereby agree to release, waive, discharge, hold harmless, defend and indemnify CRAFT IT YOURSELF, its managers, members, and employees from any and all claims, actions or losses for bodily injury, property damage, wrongful death, or otherwise, which may arise from my participation in any workshops or classes, resulting products from workshops or classes, or from use of equipment provided by CRAFT IT YOURSELF. I accept full liability for the products and/creations made by the participant in the workshop.
